How do I complete my timelogs in DocuSign for Community Health if I live in California? 

Note: This article is for students completing their timelog for Community Health who live in California. All other students should follow the instructions here.

Answer

The purpose of the timelog is to track the field experiences you are doing for the Community Health course. The timelog will help you keep track of your activities, the date completed, show the contact information (including their name, address, phone, and/or email), the hours you spent, and the relevance to the topic you chose. DocuSign is the platform where you will complete your timelog and will provide a document that is retrievable, revisable, and can be saved until signed. Please use this article to assist you in filling out your timelog in DocuSign.

ACCESSING TIMELOGS

 

  1. Click the Timelog form hyperlink in the performance assessment; this will take you to the DocuSign request form.
  2. Fill out all of the information for all signers. If you have questions about who your Clinical Instructor or Preceptor is, please contact Health Field Placement.
    1. Input your information, using your WGU email address.
  3. Click Begin Signing.

  4. You will receive an email with a Signing Validation Code and link. Use this hyperlink to add in activities as you go along, or all at once. The document associated with the URL can be edited up until it is signed. After it is signed, no edits can be made. If no email is sent, please access the timelog again to ensure it is sent to the correct email before any activities are added.
  5. Your email will contain a Signing Validation Code. This is the same as the Access Code.

  6. The following screen will show up as the Signing Validation Code is emailed to you. Input the access code from your email. This email will provide access to revise and edit the timelog prior to signing.

  7. Go back to your email and Click Resume Signing to begin filling out the timelog.

FILLING OUT THE TIMELOG

 

  1. Once you have received your access code email and accessed the timelog, please check the box to accept the Electronic Records and Signature Disclosure and then click Continue.

  2. Begin filling out the timelog, ensuring activities are appropriate per guidance on the timelog itself.
    1. Select your topic from the dropdown menu.
    2. Fill out the date of activity.
    3. Fill out the activity.
    4. Enter the activity contact information (name, physical address, phone, and/or email) in the following format:
      OC Farmers Market (line 1: location name)
      101 Market Street Santa Ana CA (line 2: location address on one line)
      714-555-5555 Contact Allen Nelson (line 3: point of contact phone number and name)
    5. Enter the total hours spent on the activity, not including preparation time.
    6. Describe the relevance of the activity to the topic you selected.

Note: It is imperative you select Finish Later while filling out your timelog to save your progress.

Note: Once you sign, you will not be able to make revisions. You will be required to input your activities again.

 

SAVING YOUR WORK AND RETURNING TO THE TIMELOG

It is imperative to save your work as you proceed by using the following instructions:

  1. To save the document, go to the top right dropdown list labeled, Other Actions.
  2. Click Finish Later.

  3. Whenever changes are needed, use the original DocuSign email you received. You can make changes as often as needed until you sign it.

SIGNING THE TIMELOG

  1. Click Finish when all activities have been entered and the total hours adds up to 90 hours.

  2. Click Sign.
  3. You can adopt the generic signature, change the style, or draw your own.
  4. Click Adopt and Sign

  6. DocuSign will offer a free DocuSign account. This is not required. You can bypass this by selecting No Thanks.
  7. After you sign your timelog it will be routed to the next signer, which is your Clinical Instructor.
    1. You will be notified via email when the signer has opened the document and also when the signer has completed signing.
    2. If there are no changes, your timelog will be automatically routed to the final signer, the preceptor.
    3. If there are changes, please refer to Revisions from a Signer section.
  8. Once the timelog is signed by all parties, you will be notified via email.
  9. You will be able to retrieve your signed timelog to upload into Taskstream with your task submission or through your course.
  10. Submit the final, signed emailed copy and upload it with your task submission in Taskstream or through your course.

 

REVISIONS FROM A SIGNER (CLINICAL INSTRUCTOR OR PRECEPTOR)

If your timelog was sent back with changes from any of the signers, you will be required to make those changes and initial them.

If you are required to make changes, an email will be sent to you with a message from the signer who requests the changes. Click Review Document to review and make changes. You may be asked to enter your original access code.

  1. When you open your timelog, you will see the line(s) that require revisions.
  2. The signer will input changes that are needed.
  3. You will be required to initial the comments and initial your changes.

  4. There will be an arrow on the line that requires revisions by the signer. At the end of the line, you will be required to initial to acknowledge the revisions.DocuSign Initial Here Revisions.jpg
  5. After you initial to acknowledge the revisions, you will make the appropriate changes.
  6. You will need to initial your changes after you complete them.
  7. After all changes have been made and initialed, select Finish.

The document will be routed to the next signers. You will get an email notification when it is received and signed by all subsequent signers.

 

COMPLETED TIMELOG RETRIEVAL

After your document has been signed by all parties, you will be notified via email. The completed timelog can be retrieved via this email and the DocuSign link. The email also contains a PDF of the completed timelog at the bottom.

RETURNED FOR REVISION FROM EVALUATION

After your timelog is signed, you are required to upload the signed timelog with your performance assessment submission. Your timelog will be evaluated with your performance assessment. If your timelog is returned for changes, you are required to fill out an addendum timelog.

Note: This only needs to be filled out if the original timelog is returned from evaluation or you have signed and cannot make changes. The link for the addendum is located in your performance assessment, below the timelog link. You are required to input the signer information, similar to when you first opened your timelog. Your addendum will need to be signed again.

 

  1. Select your original topic.
  2. Input the line that needed correction from your original timelog, including all the details.
  3. Below it, you will put the revised line.
  4. You will do this for as many lines as are required to be revised. 
    Note: It is imperative you select Finish Later while filling out your addendum to save your progress.
  5. Once you have finished, sign the addendum and it will be routed to the next signer to review and sign.
  6. You must upload your original timelog and your addendum with your performance assessment resubmission.
