Alaska Student Teacher Form Required Prior to DT

Two months prior to your Demonstration Teaching (DT) start date you must begin the process of obtaining an Alaska Student Teaching License. The application process includes completing an application form, a fingerprint based state and federal background check through the Alaska Department of Education and Early Development, and WGU completing and submitting a Program Enrollment form.

 

THE APPLICATION PROCESS

 

1) Two months prior to your anticipated DT start date, download and complete pages 1 and 2 of the Alaska Department of Education and Early Development Background Check FormHave your fingerprinting done on a Form FD-258 fingerprint card at a local law enforcement agency office, then send the prints with the completed DEED background check form and the background check fee to:

If Form FD-258 fingerprint cards are not available at your local law enforcement agency office, you can order them from the Alaska Department of Education and Early Development Teacher Education & Certification Office at tcwebmail@alaska.gov.

 

2) After your state and federal background check results are returned to the DEED and your Demonstration Teaching placement has been secured by your WGU Field Placement Coordinator, download and complete the Student Teaching Authorization Application, with the exception of page 7, the Student Teaching Program Enrollment Form, following the included instructions.

  

3) Complete the Applicant Information section at the top of the Student Teaching Authorization Program Enrollment form and submit it to the WGU Licensure Office for completion by scanning and emailing it to licensure@wgu.edu or faxing it to 801-401-7961. 

 

Please include your current mailing address becuase the completed Program Enrollment form will be mailed back to you for inclusion in your Student Teaching Authorization application packet.

 

4) Compile your completed Student Teaching Authorization Application, Passing Scores on a  Basic Competency Exam (official score report required), and your completed Program Enrollment form, and mail them to:

There is no fee for the Student Teaching Authorization application. Your application must be received by the DEED within 6 months of your background check.

 

5) Monitor the status of your Student Teaching Authorization application using the Alaska License Lookup, and when it is issued, you must notify the WGU Teachers College Background Checks Department by uploading your student teaching permit or a message it has been issued in your student Field Experience portal under DT Background Clearance.
