UT Mathematics Endorsement Application Information and Instructions

GENERAL INFORMATION

The information in this article applies to students enrolled in WGU programs that lead to a Utah Mathematics Endorsement, Level 2 or Level 4 (Master of Arts, Mathematics Education 5-9 or 5-12).

 

Information on adding endorsements in other subject areas offered by WGU can be found in the "Applying for Certification in Utah" section of the Utah Table of Contents: Teacher Licensure Articles. General information for adding endorsements in subject areas that are not offered at WGU can be found through the Utah State Office of Education.

 

APPLICATION INSTRUCTIONS

 

Graduates of WGU's Master of Arts, Mathematics Education (5-9 or 5-12) programs are eligible to add either a Level 2 or Level 4 Mathematics Endorsement to their existing Utah license. Please follow these steps to add the appropriate UT Mathematics Endorsement:

Step 1: If you graduated from the Master of Arts, Mathematics Education (5-9) program, complete the Applicant Information portion of the Application for the Utah State Office of Education Mathematics Level 2 form.  

 

If you graduated from the Master of Arts, Mathematics Education (5-12) program, complete the Applicant Information portion of the Application for the Utah State Office of Education Mathematics Level 4 form.  

 

Note: Despite the fact that fees are listed on the Application form, the Utah State Office of Education is not currently requiring fees for endorsement applications.  No check is required to be included with your application.

Step 2:  Once you have completed the Applicant Information portion of the form, please save it and attach it to an email requesting recommendation to licensure@wgu.edu. A WGU Certification Officer will finalize the application form, print and highlight an official WGU transcript, and mail them to the Utah State Office of Education on your behalf. You will receive an email notification of submission with an attached copy of your form for your records. 

 

It usually takes 2-3 weeks from the date of submission for your recommendation to be processed.
